Arts in Education - Community Consortium Final ReportArts Education Community Consortium Grants require a Final Report that consists of four components (Described in further detail below):
1. The Final Report Form (statistical and demographic questions) 2. A Narrative Evaluation (2-3 pages) 3. Final Budget (showing original projections and actuals) 4. Supplemental Materials (evaluation and documentation)
Final Reports must be received by the Arts in Education Program at the Washington State Arts Commission no later than July 30, 2008. Payment of the final grant invoice will not be made until the Final Report is received.
Please note: Grantees whose Final Reports arrive after the July 30 deadline will be subject to a 10% funding reduction on a subsequent AIE grant contract.
Please include all four components in one envelope, and mail to:

Final Report Components:
1. Final Report Form (DOC) - This form includes statistical and demographic questions required for state and federal reporting.
2. Narrative Evaluation Questions (DOC) - This page lists the questions to consider in your Narrative Evaluation.
3. Final Budget Report (XLS) - Please include a revised budget showing actual income and expenses. If possible, please start with the original budget you submitted with the grant application, and add a new column for the actual amounts spent in each category. You are encouraged but not required to use this budget form.
4. Final Report Supplemental Materials -Please submit a limited sample of supplemental materials to add depth to our understanding of your programs. At least one item from category “a” is expected; additional items from “b” and/or “c” are optional but encouraged.
Please include just enough in each category to highlight the range and quality of your program; please do not include multiple copies of similar documents or a catalogue of every component of your program.
Please note that we encourage the submission of electronic photographs so that we can use them in our program documentation. Photos should be saved as tiff or jpeg files and included on a CD with your program report. Please include captions and any necessary photographer credits. Please note: we will assume your permission to use any photos submitted with final reports for WSAC’s communications needs. If you do not have appropriate permissions from the photographer or subjects of the photos, please do not submit them.
